Test Application Forms
Only this form will be accepted from January 2013 These must be signed by the coach and the skater (parent or guardian if skater is under 18) to state that they have read the rules and regulations on the reverse and that the skater is available to take the test. Any dates when the skater is likely to be unavailable must be clearly stated on the form to avoid forfeiture of test fees.
