To download the relevant form, simply right-click on the link and 'Save Target As . . . ' which will allow you to save the file to disk. Then all you need to do is print the form, fill it out and send it to NISA at:

Test Application Forms
With effect from 20th August 2010 only the new logo version of the test application forms will be accepted. These must be signed by the coach and the skater (parent or guardian if skater is under 18) to state that they have read the rules and regulations on the reverse and that the skater is available to take the test. Any dates when the skater is likely to be unavailable must be clearly stated on the form to avoid forfeiture of test fees.
